Liability Waiver for Events & Classes
Please Note: This previews the Kitten Yoga and Wellspring Health Center liability waiver. We kindly ask that you take a moment to review it before your class. When you arrive, you will sign the official waiver. This preview is meant to help you understand the terms, allowing for a smooth and prompt start to your class.
This waiver covers all classes and events sponsored by Wellspring Health Center, 6485 City West Pkwy, Eden Prairie, MN.
Section 1: General Waiver for All Classes, Health Programs, and Events
By signing below, I understand that it is my responsibility to consult with a physician before I participate in the classes, health programs, or events. I represent that I am physically fit and have no medical condition that would prevent my full participation. I acknowledge that participation in any events, workshops, or classes provided by Wellspring Health Center, including but not limited to fitness classes, sound baths, meditation classes, massage, benefit/fundraising events, and other health services or activities, involves certain risks. These risks may include but are not limited to, bodily injury, illness, emotional distress, and property damage. I understand and agree that Wellspring Health Center, its owners, staff, and agents are not responsible for any injury, illness, or damage I may suffer because I participate in these activities, whether on-site at Wellspring Health Center or off-site at locations co-hosted with other businesses. I, my heirs, or legal representatives forever release, discharge, and hold harmless Wellspring Health Center, its owners, staff, and agents from any claims, demands, or causes of action that may arise from my participation in these activities.
I acknowledge the risks of COVID-19 and assume full responsibility for any related expenses or health issues from attending an event. I release Wellspring Health Center PLLC from all COVID-19-related claims.
- Section 2: Sound Bath Specific Waiver
Sound baths are a form of complementary medicine and should not be considered a replacement for indicated medical treatments. It is important that people who are pregnant, experience epilepsy or seizures, or have a diagnosed mental health condition consult their primary care provider before taking part in sound baths. By signing below, you understand and accept this risk and agree that Wellspring Health Center will not be liable for any injury or adverse effects sustained by you during or after participation in sound baths.
- Section 3: Kitten Yoga Specific Waiver
Interacting with The Bitty Kitty Brigade kittens involves some risk of injury or illness to you. Injuries include, but are not limited to, cat scratches and cat bites. Cat bites and/or scratches may cause infections. Most illnesses or conditions are species-specific; however, a few, such as giardia, toxoplasmosis, and other parasitic or fungal conditions, such as ringworm, are not. By signing below, you understand and accept this risk and agree that The Bitty Kitty Brigade and Wellspring Health Center will not be liable for any injury or illness sustained by you while you are involved with, in any manner, handling the kittens of The Bitty Kitty Brigade. NOTE: The Bitty Kitty Brigade strongly recommends that your tetanus vaccination is up-to-date.
