I came back to Wellspring 10 years after a prior successful rehabilitation. I have had shoulder pain since an injury about five years ago. As an active watersports enthusiast, the shoulder pain limited my ability to waterski and wakeboard. Before coming to Wellspring, I went to an orthopedic center for X-rays, and they said there was no problem, to rest and take ibuprofen. But I could barely bend my knee, making it difficult to get in and out of the car, step into the tub, and go up and down steps. I got to the point where I couldn't even turn my head to one side. I couldn't sit up or roll over in bed. I could barely look over my shoulder to change lanes while driving. I was in pain every time I used my right hand and arm. I had pain with most of my daily activities and started using my left hand for most tasks due to extreme pain. I've had lower back pain for a while and it definitely made me grouchy! I also had a bad ankle sprain from a few years ago, and a newly developed lower quad discomfort/pain. The lower quad pain made it difficult to enjoy my favorite hobbie, pickle ball. Due to the pain, it also caused me to be irritable at work. I came because I was having severe pain in my shoulder that wouldn't go away even though I iced it, did neck stretches and let it rest for a few weeks. I couldn't chop vegetables for meal prep, nor reach the top of my head to shampoo my hair, I couldn't style my hair, it hurt to move my shoulder. I had to buy pre-chopped vegetables and ingredients that could be poured together without much prep work. My family had to put pans in the oven for me. What payment methods do you accept?

We accept most health insurance plans and file the claims for you. Most patients will have an out-of-pocket amount due such as a deductible, co-pay, or co-insurance. Some services (like massage and SoftWave therapy) are self-pay and need to be paid for directly at the time of service.

How do I come prepared to my appointment?

Please have your insurance card available when you call us. This will allow us to verify your insurance benefits and have the details ready for review on your first visit.

To bring on your first visit:

In most cases, a doctor’s referral for physical therapy isn’t needed since Wellspring has a medical specialist onsite to evaluate, diagnose, and prescribe your therapy. But if you have already obtained a physical therapy prescription from another medical physician, please bring it to your initial visit so we can incorporate it into your treatment plan and keep your physician updated on your progress.
In case of an automobile accident or worker’s compensation claim, also bring any case manager or insurance adjuster contact names, phone numbers, and claim number.
Please complete Patient Forms here. If you are unable to do this, please arrive 15 minutes early to complete forms.
Please wear comfortable clothing that allows easy access to your problem areas.

Does health insurance cover your services?
