Our staff will assist you with the proper steps to open an auto accident claim, or you can call your insurance agent before your visit and notify them that you will be seeing a physical therapist and massage therapist and request that they open a claim. Our office will contact your insurance company and get all the information needed to file your claims. Our integrative team of medical, physical therapy, and massage therapy providers will evaluate and document your injuries. If indicated, they may begin treatment to help you feel better.
Forms To Complete To Open a Claim
Once you call your agent and request that a claim is opened for health care expenses, your insurance company will assign a “claims adjuster”. The claims adjuster is the person who processes and pays any claims to your health care providers and monitors the type and frequency of any treatment you receive. For a claim to be opened, you will be sent specific forms to complete:
- Application for Benefits
- Authorization for Release of Medical Records
- Authorization for Wage and Salary Information
These forms should be completed and mailed back to the claims adjuster as soon as possible. If you have retained a personal injury attorney, they will assist you in completing these forms. Otherwise, our experienced staff can help you in completing these forms.
Mileage Reimbursement
Minnesota No-Fault Law allows reimbursement for mileage to get to your treatment. To be reimbursed for mileage, you must submit a document stating the dates and mileage of each visit.
Reimbursement for Replacement of Services
Replacement services include those expenses incurred to obtain the usual and necessary services for activities that the injured person would have otherwise been able to perform. For example, if the injured person is unable to do household activities (cleaning, laundry) due to medically documented restrictions, these activities may be reimbursable.
Reimbursement for Wage Loss After an Auto Accident
Minnesota No-Fault Law allows for wage loss benefits of 85% of the actual wage loss up to a maximum of $250.00 per week until you can return to work.
