Everything You Need Before Your First Visit

If you have booked your first consultation or are getting close to it, this page walks you through what to expect, what to bring, and what to know before you arrive. The goal is for you to walk in calm and prepared.

BEFORE YOUR VISIT

Three things to take care of in advance.

01

Online intake forms

After you book, we will send you a link to complete intake paperwork online before your visit. This saves time at the clinic and helps your provider arrive prepared. Most patients finish the online intake in 10 to 15 minutes.

02

Insurance verification

If you are using insurance, we verify your benefits before your first visit so you walk in knowing what is covered. If you are using auto insurance or workers compensation, we handle that intake separately so it does not slow your appointment down.

03

Arrive 10 minutes early

We recommend arriving about 10 minutes before your appointment. That gives you time to settle in, complete any final paperwork that was not done online, and start your visit relaxed rather than rushed.

What to expect.

Your first visit is a 60-minute consultation. It starts with a real conversation about your history. How long this has been going on. What you have already tried. What is getting in the way of your daily life. We may do a brief hands-on evaluation, explain what they are seeing in plain language, and walk through a recommended plan. The visit is unrushed by design. You will leave with clear next steps and realistic expectations.

VISITING US

Parking, accessibility, and transit.

Parking

Free parking is available directly outside the clinic. The lot is well-lit, easy to access, and we have never had a patient struggle to find a spot during business hours.

Accessibility

The clinic is wheelchair accessible with no steps from the parking lot to the front door. If you have specific accessibility needs, call ahead at (952) 933-1150 and we will make sure your visit is as easy as possible.

Public transit

If you are coming by bus or rideshare, our location is accessible from major routes through Eden Prairie. The front desk can help with directions if you call ahead.

COMMON QUESTIONS

Questions from new patients.

Bring your insurance card so we can verify your benefits. If you have already seen another provider for this issue and have a referral or prescription, bring that too. If you were in an auto accident or have a workers’ compensation claim, bring your claim number and any adjuster contact information. Arrive 15 minutes early if you have not already completed your intake forms online.

Your first visit typically lasts between one hour and one and a half hours. This allows time for a thorough consultation, a review of your history, and the start of your care plan. Follow-up appointments are typically one hour.

Wear comfortable, loose-fitting clothing that allows your therapist to access the area being treated. If you are coming for hip, knee, or leg treatment, shorts are helpful. For shoulder or neck treatment, a tank top or loose shirt works well. We have changing rooms if needed.

Your first visit starts with a thorough consultation. We listen to your full history, including what you have tried before and what has or has not worked. By the end of the visit, you will have a clear understanding of what to expect going forward.

Your first visit is a free consultation where we take the time to really understand what is going on. We listen to your full history, ask about what you have tried, and explain what we are seeing. In some cases, your nurse practitioner may begin your evaluation or take X-rays at the same visit. Treatment typically begins at your next appointment, once your care team has reviewed everything and built a plan around your specific situation. You will leave your first visit knowing exactly what to expect.

We have online intake forms you can complete before your visit at wellspring-hc.com/new-patients/. Completing them in advance saves time and helps your care team prepare before you walk in. If you cannot complete them ahead of time, arrive 15 minutes early.
